Prosperity

Summary

PatientPartner will bring prosperity to underserved communites by providing free access to the PatientPartner platform for 5000 patients in designated HPSA zones by the end of 2024.

Activities & Initiatives

PatientPartner will work with community engagement centers and deploy various marketing initiatives to help bring awareness of this free program. In addition, we will work with HCP offices to make their practices available for those in various HPSA's (Health Professional Shortage Area) areas by the end of 2024.

Measures & Targets

PatientPartner will measure success of this iniatiave by tracking patient zip codes, how many HCPs we activate on the platform, and how many of those patients seek care with those HCPs.
